CONSENT TO THE PROCESSING OF PERSONAL DATA

adapted to the provisions of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, (GDPR)

List of personal data:

Personal data of applicants for inclusion in the database for sending newsletters - email address.

Purposes of personal data processing:

Evidence of e-mail addresses for sending marketing offers.

Personal data may not be further processed in a way incompatible with those purposes.

Recipients of personal data: controller only

Legal basis for the processing of personal data - Article 6, para. 1. letter a) GDPR - the data subject has consented to the processing of his or her personal data for at least one specific purpose.

Retention period of personal data - 10 years.

The data subject hereby declares on his / her honor that he / she has given the controller his / her express and unconditional consent to process his / her personal data to the above extent, for the stated purpose and during the above period.

The data subject is obliged to provide true and current personal data. In the event of a change in personal data, the data subject is obliged to notify the controller of the change without delay.

The person concerned has the right to withdraw his or her consent at any time. Withdrawal of consent shall not affect the lawfulness of the processing resulting from the consent prior to its withdrawal.

The rights of the data subject are defined in Chapter 3 of the GDPR. It goes e.g. o the right to request from the controller access to personal data concerning the data subject, the right to rectify or delete them or to limit processing or the right to object to the processing, as well as the right to data transfer and the right to complain to the supervisory authority.
